DZA Registration Form Important Disclosures

Standard Individual Participant Institute Tuition & Fees: $985.

Includes: reception at Dinah's home, 6 meals, relevant resource books for the course, a research white paper, overhead transparencies, duplicable handouts, a course notebook, construction materials and tools, plus a video of each teacher participant presenting a foldable lesson as well as the corresponding foldables. 20 hours of direct classroom instruction are involved.

1. We require 50% of your tuition to hold your spot in an Institute. This must be paid at the time of your registration and is payable to: Dinah Zike Academy
   Acceptable forms of payment include: Credit Card (MasterCard or Visa only), Personal Check, Approved Purchase Order, Cashiers Check, Money Order
   DZ reserves the right to charge your credit card the correct amount if there is an error in calculation of amount due.
   Final Payment is due two weeks (14 calendar days) before 1st day of Institute.
2. Billed Purchase Order Policy: DZA accepts purchase orders as payment for 50% pre-registration or final registration payment. Purchase orders must be signed by issuer, included with registration form and received at DZA at least 14 calendar days before Institute start date.
3. Do not finalize travel or hotel reservations until we have informed you that the Institute you selected has both reached the minimum number of participants and has not exceeded the maximum.
4. Should 1) the institute not make the minimum or 2) you have to cancel, up to two weeks (14 calendar days) before your registered Institute Start Date, we will gladly apply the required minimum as a credit toward an upcoming institute of your choosing within two year period.
5. Smoking Policy: Smoking is not allowed inside the Dinah Zike Academy. There is a designated Outdoor Smoking Area.
6. We will attempt to accommodate known physical challenges, dietary restrictions and other special needs, but we need to know about these in advance, so that we can inform you of what we can and cannot provide.
7. Continuing Education Credits through School Districts - You will be given a Certificate of Attendance indicating that you have completed 20 hours of direct instruction during an institute. It is the registrant's responsibility to see if and how this will apply as Continuing Education Credits though your district.
8. Travel Arrangements and Lodging Accommodations must be made by the Individual Registrant.
9. I agree that my image, and likeness, as shown in DZA videotape, photographs, film, and/or electronic images in which they appear, and/or audio recordings made of my voice may be used by Dinah-Might Adventures, L.P., its assigns or successors, in whatever form they desire.
